We’re currently recruiting for a Chronic Disease Champion in Aldershot, Hampshire, for a high-QoF achieving Practice. The hours on offer are part time, and you will be well-supported by the friendly and approachable GP and management team. The surgery takes pride in it’s fantastic cloe-knit inner-network, and it’s a pleasant and positive environment to work in.

The post is offering some great employee benefits such as a top band 6 salary (in the region of £17-£19 per hour dependent on experience), 25 days annual leave plus bank holidays, and training and progression opportunities to develop your nursing career and CPD.

To be considered for this progressive opportunity, you will be trained in all areas of Chronic Diseases, and able to carry out reviews mainly in Diabetes and Respiratory. Further Treatment Room skills would also be required such as Immunisations and Cytology and Prescribing would be a bonus.
